Integrated Care Navigation
Our Integrated Care Team is dedicated to providing a holistic, well-coordinated approach to healthcare in partnership with internal and external providers and services to care for our patients as a whole. Coordination of care within the team includes managing referrals with specialists, providing community resources, communicating with local hospitals and medical community to manage hospitalizations and ER visits, requesting and obtaining medical records for continuation of care, and working with patients to ensure their health goals are met.
